What is oculodermatology?

The skin of the eyelids is the thinnest and most delicate on the human body. Its proximity to the eye and its protective, mobile role give it unique characteristics. Oculodermatology provides specialized care for all conditions affecting the eyelid skin and periorbital region, whether dermatological, inflammatory, infectious, or aesthetic.

Ocular Rosacea

Rosacea is a chronic inflammatory disease of the facial skin that can extend to the eyelids (palpebral rosacea) and the ocular surface (ocular rosacea). Involved factors include vascular predisposition, local immune abnormalities, skin flora (e.g., Demodex), and triggering factors (e.g., alcohol, hot food, sun).

Redness, burning, itching, ocular irritation, photophobia, tearing, recurrent chalazia

Risks

Eyelid hygiene, lubricating eye drops, topical or oral antibiotics, control of triggering factors

Pathology

Palpebral Eczema

Palpebral eczema is an inflammation of the eyelid skin, which can be atopic, irritative, or allergic in origin. This dermatitis manifests as redness, edema, and itching. Appropriate management, including prevention and local care, is essential to limit recurrences and complications.

Causes / Triggers

Atopy, chemicals, soaps, cosmetics, perfumes, eye drops, contact lenses

Clinical Signs

Redness, peripalpebral edema, intense pruritus, fine desquamation, fissures, sometimes cheilosis, and highly reactive eyelid skin

Risks

Chronic dermatitis, hyperpigmentation, skin atrophy in the case of inappropriate prolonged corticosteroid therapy

Management

Hypoallergenic products, prior testing of new cosmetics, attention to eye drops (prefer preservative-free formulas)

Pathology

Palpebral Cutaneous Herpes

Palpebral cutaneous herpes is a viral infection generally caused by HSV-1, which can recur after an initial episode. It manifests as painful vesicles on the eyelid and carries a risk of corneal involvement. Prompt and appropriate management helps limit complications and recurrences.

Causes / Triggers

HSV-1 reactivation after primary infection, stress, fatigue, sun exposure

Clinical Signs

Prodrome of tingling and burning, grouped vesicles, crusts on the eyelid

Risks

Herpetic keratitis, palpebral or corneal scars, frequent recurrences

Management

Clinical diagnosis, if in doubt, PCR or viral swab, ophthalmological examination, prophylactic treatment

Pathology

Blepharitis

Blepharitis is a chronic or recurrent inflammation of the eyelid margins. It most often results from dysfunction of the Meibomian glands, responsible for the lipid secretion of the tear film; but can also be linked to an infection (often staphylococcal, Demodex), seborrhea, or dermatological conditions such as rosacea.

Causes / Triggers

Meibomian gland dysfunction, infections (staphylococci, Demodex), seborrhea, rosacea

Clinical Signs

Redness, eyelid thickening, itching, burning, gritty sensation, scales, eyelash loss

Risks

Tear film instability, dry eye, recurrent styes or chalazia, keratitis, corneal erosions or ulcerations, tired appearance of the eyes

Management

Regular eyelid hygiene, warm compresses, local antibiotics if necessary, treatment of associated conditions
